An infant with recurrent vomiting

An infant with recurrent vomiting was found to be anemic and have an acutely raised serum creatinine. A blood picture showed evidence of red blood cell fragmentation with multiple Burr cells. He was also found to have thrombocytopenia and an abnormal platelet function. These are features most typical of
