An 11 year old girl known asthmatic is brought to the Casualty Department with an exerbation of her asthma. She is noted to have an audible wheeze which is also present on auscultation. She is able to talk in phrases but not in long sentences and is noted to be using her accessory muscles of respiration . There is no central cyanosis. Oximetry reveals an oxygen saturation of 94% and a peak flow of 60% of the predicted value. Appropriate management would include
a) iv theophylline
b) Nebulised ipratropium bromide
c) Monoclonal Anti-IgE Antibody
d) Single dose 1 ml 0.5% salbutamol
e) Nebulised beta2 agonist 3 doses at 20 min. intervals
